Job Summary

We are seeking a highly skilled and detail-oriented Intake Coordinator to join our team at Universal Health Services. As an Intake Coordinator, you will play a critical role in coordinating the referral development and pre-admission patient assessment process.

Key Responsibilities
  • Respond to Intake Calls and Complete Admission Paperwork: Respond to incoming calls from patients and families, and complete all necessary admission paperwork in a timely and accurate manner.
  • Facilitate Groups and Sessions: Facilitate groups, family, and individual sessions, and complete all necessary documentation.
  • Schedule Unit Assessments and Meetings: Schedule unit assessments, meetings, and appointments as needed.
  • Utilize Assessment Tools: Utilize assessment tools, including the Geriatric Depression Scale, Zung, SLUMS, and alcohol screening tools, to assess patient needs.
  • Assist with Discharge Planning: Assist with discharge planning, connecting patients with day treatment, outpatient medication management, therapy appointments, residential treatment, and peer support services.
  • Communicate with Referral Sources: Communicate with referral sources, including family members, legal system, psychiatrists, primary care physicians, and other treatment facilities.
  • Review Patient Accounts: Review newly admitted patient accounts for eligibility and authorization daily.
  • Maintain Schedule for Filing Court Paperwork: Maintain a schedule for filing court paperwork and hearings, and work closely with court personnel to maintain good relationships.
  • Participate in Interdisciplinary Team Meetings: Participate as an active member of the interdisciplinary team, addressing issues in a timely manner, including crisis and higher-level-of-care referrals.
  • Maintain Confidentiality and Professional Boundaries: Maintain confidentiality and professional boundaries with clients, family members, and other staff members.
  • Represent the Program: Appropriately represent the program at public and private outreach events.
  • Attend Unit and Hospital Meetings: Attend unit and hospital meetings as directed.
  • Identify Areas for Improvement: Identify areas needing improvement and utilize the facility's performance improvement process.
  • Enter Data into HorizonPlus System: Enter required data into the HorizonPlus system and generate monthly reports to track and trend referral, admission, and discharge data.
  • Utilize Data to Complete Reports: Utilize data to complete program key statistical reports and generate reports as needed for the SVPO, VPCE, program director, and client hospital leadership.
Requirements
  • Education: Bachelor's degree from an accredited college with a major in chemical dependency, psychology, social work, counseling, nursing, or other related field.
  • Experience: Minimum three years' experience in a similar position and/or industry.
  • Licensure: LMHC or LCSW, MSW licensure (LCSW preferred).
  • Skills: Proficient in software applications, excellent written and verbal communication and presentation skills, ability to read and comprehend moderate to complex instructions and correspondence, ability to write detailed correspondence, and ability to effectively present information in one-on-one and small group situations.
