Job Summary

We are seeking a highly skilled and compassionate Intake Services Coordinator to join our team at UHS. As a key member of our healthcare team, you will play a critical role in coordinating the referral development and pre-admission patient assessment process.

Key Responsibilities
  • Respond to intake calls and complete admission paperwork
  • Facilitate groups, family, and individual sessions, and complete all necessary documentation
  • Schedule unit assessments, meetings, and appointments as appropriate
  • Utilize assessment tools, including the Geriatric Depression Scale, Zung, SLUMS, and alcohol screening tools
  • Assist in completing the biopsychosocial assessment and discharge planning
  • Communicate with referral sources, including family members, legal systems, psychiatrists, primary care physicians, and other treatment facilities
  • Review newly admitted patient accounts for eligibility and authorization daily
  • Maintain a schedule for filing court paperwork and hearings, and work closely with court personnel to maintain good relationships
  • Participate as an active member of the interdisciplinary team and address issues in a timely manner, including crisis and higher-level care referrals
  • Maintain appropriate ethical and clinical boundaries with clients, family members, and other staff members
  • Maintain compliance with state and federal confidentiality laws, as well as UHS's confidentiality policies and procedures
  • Maintain a professional code of conduct as established by UHS's Code of Conduct
  • Represent the program at public and private outreach events
  • Attend unit and hospital meetings as directed
  • Identify areas needing improvement and utilize the facility performance improvement process
  • Actively participate in department processes as requested
  • Enter required data into the HorizonPlus system and generate monthly reports to track and trend referral, admission, and discharge data
  • Utilize data to complete program key statistical reports and generate reports as needed for the SVPO, VPCE, program director, and client hospital leadership
  • Promote and develop positive, professional interactions with patients and their families, peers, leadership, clients, etc.
  • Correct issues identified and report errors to the Program Director
  • Assist in submission of reports and requests for documents from various regulatory agencies
  • Prepare and disseminate monthly reports and complete required follow-up on activities on behalf of the Director
  • Prepare and submit all operational tasks/documentation as required by the corporate offices
  • Regularly collect data to provide input and/or feedback to appropriate customers/suppliers to improve work
Requirements
  • A Bachelor's Degree from an accredited college with a major in chemical dependency, psychology, social work, counseling, nursing, or other related field
  • A Master's Degree or above from an accredited college with a major in chemical dependency, psychology, social work, counseling, nursing, or other related field, which includes a practicum in a health or human service agency
  • LMHC or LCSW licensure
  • LCSW preferred
  • Minimum three years' of experience in a similar position and/or industry
  • Proficient in software applications
  • Excellent written and verbal communication and presentation skills
  • Ability to read and comprehend moderate to complex instructions and correspondence
  • Ability to write detailed correspondence
  • Ability to effectively present information in one-on-one and small group situations to patients and their families, program leadership, employees, clients, providers, vendors, etc.
