Join Our Team and Make a Difference

We're seeking a dedicated and compassionate Behavioral Health Professional to join our Integrated Care Team. As a member of our team, you'll work collaboratively with healthcare, behavioral health, and wellness providers to ensure that all services prescribed in the Integrated Service Plan are implemented. Your responsibilities will include:
  • Developing and maintaining the Integrated Service Plan (ISP) within required time-frames.
  • Participating in the development of the integrated comprehensive assessment and Integrated Service Plan (ISP).
  • Implementing services to members in accordance with their plan and providing ongoing care and reviews progress.
  • Coordinating with all involved healthcare, behavioral health, and wellness providers and maintaining current documentation on members' progress in managing chronic medical and psychiatric conditions and assisting the individual in achieving their recovery goals.
  • Ensuring compliance and adequate services, additional job duties may be required to meet the needs of the program and/or department.
