Nurse Practitioner or Physician Assistant

At Optum, we're revolutionizing the way healthcare is delivered. Our Housecalls team is seeking a skilled Nurse Practitioner or Physician Assistant to join our mission of providing high-quality, patient-centered care in the comfort of patients' homes.
Key Responsibilities:- Conduct comprehensive in-home assessments, including evaluations of past medical history, review of systems, medication reconciliation, vital signs, and physical exams.
- Perform evidence-based practice screenings, including point-of-care testing as appropriate.
- Identify diagnoses and develop care management plans to address health risks and improve patient outcomes.
- Communicate findings with patients' primary care physicians to ensure seamless care coordination.
- Provide education and support to patients on disease management, medication adherence, and healthy lifestyle choices.
- Active NP license or ability to obtain by start date.
- ANCC or AANP national certification in Family, Geriatrics, Adult, or Adult-Gerontology Primary Care, or ability to obtain by start date.
- Ability to complete physical requirements of the job, including lifting a 30-pound bag and navigating various dwelling conditions.
- Reliable transportation for home visits.
- Experience in gerontology, cardiology, internal medicine, or endocrinology.
- Home health care or home visit experience.
- Fluency in Spanish, Cantonese, Korean, Vietnamese, Polish, or other languages.
