**Job Summary:**
As a Social Services Assistant, you will provide emotional support to residents and their families, conducting assessments, assisting with admissions, transfers, and discharges, and referring them to appropriate support services. You will also monitor and report changes to team members and family as needed.

**Work Schedule:**
This position works regular business hours, Monday through Friday.

**Requirements:**
* Bachelor's Degree in Social Work or a related human services field
* Minimum one year supervised social work experience in long-term or sub-acute care rehabilitation
* Current knowledge of state and federal laws and regulations applicable to social work practice in long-term care
* Preference for a current Social Work license in the state

**Benefits:**

Full-time employees are eligible for tuition reimbursement after one year of employment, with a maximum reimbursement of up to $1,000 per year. Part-time employees are eligible after two years of employment, with a maximum reimbursement of up to $750 per year.
