Job Summary

We are seeking a highly skilled and compassionate Case Manager/Service Advisor to join our team at the Kentucky River Area Development District. As a key member of our staff, you will play a vital role in coordinating Participant Directed Services (PDS) and providing exceptional support to our clients.

Key Responsibilities
  • Develop and Implement Care Plans: Collaborate with clients, healthcare providers, and other stakeholders to create personalized care plans that address their unique needs and goals.
  • Case Management and Coordination: Provide ongoing case management and coordination services to ensure seamless delivery of care and support to our clients.
  • Communication and Collaboration: Establish and maintain effective working relationships with clients, families, healthcare providers, and other stakeholders to ensure comprehensive care and support.
  • Documentation and Reporting: Maintain accurate and timely records of client interactions, care plans, and service delivery, as well as prepare reports and statistics as required.
Requirements
  • Education and Experience: An Associate degree in Social Work, Health, or Human Services field, or a Bachelor's degree in a related field, with at least one year of experience in a health or human services field.
  • Skills and Qualifications: Possess a good working knowledge of computers and software applications, as well as excellent written and oral communication skills. Ability to establish and maintain effective working relationships with clients and stakeholders.
