Job Summary:

We are seeking a skilled Discharge Planner to join our team at Appalachian Regional Healthcare. As a Discharge Planner, you will play a critical role in ensuring the smooth transition of patients from acute care to community-based care. Your expertise will help us provide high-quality, patient-centered care that meets the unique needs of our patients and their families.

Key Responsibilities:

* Perform comprehensive assessments to identify patient needs and develop individualized discharge plans
* Collaborate with healthcare teams to ensure continuity of care and coordinate post-acute services
* Provide education and support to patients and families to promote self-management and independence
* Facilitate communication between healthcare providers and community resources to ensure seamless transitions
* Monitor and evaluate the effectiveness of discharge plans and make adjustments as needed

Requirements:

* Bachelor's degree in Social Work, Psychology, Sociology, or a related field
* Experience in case management or a related field
* Strong communication and interpersonal skills
* Ability to work independently and as part of a team
* Proficiency in Windows, Word, and other basic computer skills
