Discharge Planner

We are seeking a skilled Discharge Planner to join our team at Appalachian Regional Healthcare. As a key member of our healthcare team, you will play a critical role in ensuring a smooth transition of patients from acute care to community-based care.

• Conduct thorough assessments to identify patients' needs and develop personalized discharge plans.
• Collaborate with healthcare providers, patients, and families to ensure continuity of care and effective communication.
• Coordinate with community agencies and healthcare providers to arrange for post-discharge care and services.
• Provide education and support to patients and families to promote healthy behaviors and self-management.
• Monitor and evaluate the effectiveness of discharge plans and make adjustments as needed.

• Bachelor's degree in Social Work, Psychology, Sociology, or a related field.
• Experience in case management or a related field preferred.
• Strong communication and interpersonal skills.
• Ability to work independently and as part of a team.
• Proficiency in Windows, Word, and other basic computer skills.
