HopeHealth is in search of a dedicated and knowledgeable Palliative Care Nurse Practitioner to join our esteemed affiliate, HopeHealth Hospice & Palliative Care.

This part-time role offers eligibility for benefits and may require occasional weekend and holiday shifts, limited to once per quarter.

The Palliative Care Nurse Practitioner collaborates closely with referring physicians and relevant specialists to alleviate suffering for patients and their families who are dealing with serious health conditions. The practitioner will conduct assessments and provide recommendations aimed at easing distressing symptoms such as pain, shortness of breath, anxiety, and fatigue. They will also communicate difficult news compassionately and assist patients and families in setting clear and compassionate care goals.

The ideal candidate will possess strong collaborative skills, working effectively with patients, families, and referring entities, while serving as a resource in hospice and palliative care. This position will primarily serve the Southern Rhode Island area.

Key Responsibilities

Direct Patient Care
  • Conduct initial and follow-up consultations for adults with serious illnesses across various settings, including homes, assisted living facilities, nursing homes, hospitals, and clinics.
  • Perform comprehensive pain evaluations and develop management strategies for patients.
  • Evaluate and recommend interventions for non-pain symptoms such as nausea, fatigue, and anxiety.
  • Assess the patient's disease progression and co-existing conditions, collaborating with treating physicians to establish prognosis.
  • Act as a seasoned consultant, fostering positive relationships with referring medical providers.
  • Based on prognosis and patient and family goals, provide recommendations regarding treatment options and end-of-life care.
  • Facilitate counseling and care coordination among patients, families, providers, and facilities to enhance clinical outcomes.
Professional Standards
  • Demonstrate a thorough understanding of HopeHealth Hospice & Palliative Care policies and relevant State and Federal regulations.
  • Ensure accurate documentation and timely billing submissions.
  • Consistently uphold the values of HopeHealth in all work aspects, adhering to established policies and quality assurance standards.
  • Serve as a subject matter expert in hospice and palliative medicine, providing both informal and formal education to peers and new hires.
Qualifications
  • Graduated from an accredited master's or clinical doctorate program in nursing.
  • Certification as a Family/Geriatric or Adult Nurse Practitioner and licensure as a Registered Nurse Practitioner in the state of Rhode Island is required.
  • A minimum of one to three years of experience as a nurse practitioner or physician's assistant, preferably in palliative care, with at least two years in geriatrics, acute care, hospice, or oncology.
  • Experience in pain and symptom management is highly desirable.
  • Familiarity with third-party reimbursement processes is beneficial.
  • Prior experience in hospice and palliative care is preferred, along with certification in advanced practice in this field.
  • Current licenses in Rhode Island and Massachusetts as a Registered Nurse and Nurse Practitioner or Physician's Assistant.
  • Valid driver's license and a reliable, insured vehicle.
