Job Summary

UnityPoint Health is seeking a skilled RN-Care Coordinator to join our team. As a Care Coordinator, you will play a vital role in integrating and coordinating the clinical care of individuals, ensuring seamless transitions across the healthcare continuum.

Key Responsibilities
  • Screen and assess adult Medical/Surgical in-patient and observation patients to identify needs and make referrals to appropriate multi-disciplinary services.
  • Prioritize patients for care coordination based on defined criteria and monitor and coordinate an interdisciplinary plan of care in partnership with the individual and their support services.
  • Partner with the Social Work team to develop and implement effective discharge plans for complex care patients and work with internal and external resources to coordinate timely and safe transitions of patients to the appropriate level of care.
  • Participate in daily rounds, planning delivery, and evaluation of patient-focused care for prioritized patients and document the case management plan to include clinical needs, barriers to quality care, and effective utilization of resources.
  • Collaborate with patients, caregivers, internal/external healthcare providers, agencies, and payers to assist in planning and executing safe discharges and advocate for both the patient and the payer to facilitate positive outcomes.
Requirements
  • Graduate of an accredited nursing program (BSN within 5 years of hire)
  • Two years of clinical experience in focused areas working with multidisciplinary teams
  • Current licensure in good standing to practice as a Registered Nurse in Iowa
  • Certification in Case Management
