The estimated salary for this position is $42,000 - $62,000 per year, depending on qualifications and experience.

Job Description

The Community Inclusion Coordinator will be responsible for developing and supporting programming for clients to help them achieve goals that will contribute to creating the life they want with support through adult programming.

Our program helps individuals develop independent living skills, engage in activities they are interested in, and navigate the community in which they live to make these opportunities sustainable.

Responsibilities
  • Provide case management and skills training support to clients in areas such as career exploration, hard and soft employment skills, college campus inclusion, communication, decision-making, social skills, transportation goals, community safety, and activities of daily living.
  • Develop programming and curriculum on a case-by-case basis that allows opportunities for clients to develop skills to achieve their goals in a safe and controlled environment.
  • Review case records and programming to ensure compliance with Youre With Us Standards.
Requirements
  • Associates Degree in Human Services or related field, or 3-4 years of supervisor experience within a day or residential program for individuals with disabilities.
  • Direct Care experience working with individuals with Intellectual/Developmental Disabilities.
  • Excellent organizational, communication, and writing skills.
  • Demonstrated ability to work both independently and collaboratively with a culturally diverse population of individuals, co-workers, care providers, family, community members, and others.
