Job Summary:

As a Clinical Pharmacist at SSM Health, you will play a vital role in providing pharmaceutical care to patients, verifying medication orders, and dispensing pharmaceuticals in accordance with federal, state, and local regulations and hospital policy.

You will serve as a resource to other healthcare professionals, including pharmacy students and residents, and ensure that patient care meets standards of safety and effectiveness.

Overseeing the duties of pharmacy technicians and interns is also a key responsibility.

Responsibilities:

  • Ensures safety and appropriateness of medication therapy through order verification, dispensing, home medication reconciliation, chart review, patient counseling, lab value monitoring, and dose/therapy adjustment as applicable.
  • Documents medication-related plans and interventions in the electronic medical record.
  • Provides clinical consultation, delivering best-practice, evidence-based, and complete information to practitioners, making recommendations, and encouraging safe and cost-effective pharmacotherapy for patients through multi-disciplinary collaboration.
  • Oversees the medication dispensing process to ensure accurate and timely delivery of optimal pharmacotherapy to patients, including supervision of pharmacy technicians and/or interns who support the role of the pharmacist in the drug delivery system.
  • Precepts pharmacy students and residents, as applicable.
  • Works in a constant state of alertness and safe manner.
  • Performs other duties as assigned.

Requirements:

  • Pharmacy degree (Pharm.D; Bachelor of Pharmacy) from an accredited school of pharmacy.
  • No experience required.

Physical Requirements:

  • Frequent lifting/carrying and pushing/pulling objects weighing 0-25 lbs.
  • Frequent sitting, standing, walking, reaching, and repetitive foot/leg and hand/arm movements.
  • Frequent use of vision and depth perception for distances near (20 inches or less) and far (20 feet or more) and to identify and distinguish colors.
  • Frequent use of hearing and speech to share information through oral communication. Ability to hear alarms, malfunctioning machinery, etc.
  • Frequent keyboard use/data entry.
  • Occasional bending, stooping, kneeling, squatting, twisting, and gripping.
  • Occasional lifting/carrying and pushing/pulling objects weighing 25-50 lbs.
  • Rare climbing.

Professional Licenses and Certifications:

  • Pharmacist, Registered - Illinois Department of Financial and Professional Regulation (IDFPR).
  • Pharmacist - Missouri Division of Professional Registration.
  • Pharmacist - Oklahoma State Board of Pharmacy.
  • Pharmacist - Wisconsin Department of Safety and Professional Services.
