Job Summary

The Clinical Care Specialist - Home Infusion provides clinical care and education to home-based patients receiving parenteral medications dispensed from Optum Infusion Pharmacy. As a key member of the multidisciplinary team, you will work collaboratively with patients, families, and healthcare providers to ensure safe and effective infusion therapy delivery.

About the Role:

  • Establish and maintain positive relationships with patients and their caregivers to provide comprehensive care and support.
  • Assess patient appropriateness for home infusion therapy, including safety evaluations and documentation.
  • Obtain and document patient medical history, particularly as related to the diagnosis being treated with prescribed infusion therapy.
  • Complete all required clinical documentation thoroughly and timely, including nursing plans of care and visit notes.
  • Participate in case conferences to comprehensively plan patient care with the interdisciplinary team.

Responsibilities:

  1. Provide skilled care to patients of varying ages and medical conditions, as outlined in the plan of care and in accordance with physician orders.
  2. Administer medications as prescribed, performing therapy-specific clinical assessments in accordance with Optum Infusion Pharmacy policies and procedures.
  3. Communicate observations of patient response to therapy and overall physical assessment/condition with members of the care team in a timely manner.
  4. Collaborate with home health agencies (HHAs) delivering services on behalf of or in conjunction with Optum Infusion Pharmacy, providing agency education and support as requested.
  5. Maintain a comprehensive knowledge base of infusion therapies administered in the home and ambulatory infusion center, utilizing Optum Infusion Pharmacy policies and procedures in addition to all available training resources.
