Key Responsibilities
  • Coordinate care transitions for patients through assessment, planning, and implementation of appropriate services and resources.
  • Collaborate with multiple disciplines, both internal and external to the organization, to effect quality and cost-effective management of patient care along the continuum.
  • Communicate effectively with patients, families, and healthcare providers to ensure seamless care coordination.
Requirements
  • Bachelor's degree in a healthcare-related field or equivalent experience.
  • At least 3 years of experience in care management or a related field.
  • Strong analytical and problem-solving skills, with the ability to work in a fast-paced environment.
