NHTD/TBI Service Coordinator Job Description

The NHTD/TBI Service Coordinator will play a vital role in assisting applicants and participants in obtaining and coordinating the necessary services to remain in or return to their home. This position will serve as the primary contact for the participant in the Waiver Program, providing ongoing assessment and reevaluation of the participant's health and welfare, and determining the services needed to maintain the individual within their home.

Key Responsibilities:
  • Service Plan Development: Develop and implement individualized service plans, including completion of Individualized Service Plans and Plans of Protective Oversight (PPO), to ensure participants receive the necessary services.
  • Participant Advocacy: Act as the participant's advocate, providing unbiased assistance in selecting providers and locating community resources.
  • Waiver Contact List Management: Provide and review the Waiver Contact List to ensure it is accurate, up-to-date, and redistributed as needed.
  • Service Coordination: Coordinate and schedule waiver and non-waiver services to ensure timely implementation of the approved service plan.
  • Communication and Collaboration: Maintain consistent and open communication with the RRDS and waiver participants, and collaborate with team members to ensure effective service delivery.
Requirements:
  • Education: Bachelor's degree with two years of related experience in service coordination and information, linkages, and referrals for community-based services for individuals with disabilities and/or seniors.
  • Knowledge and Skills: Knowledge of waiver services and current knowledge of informal supports within the participants' communities.
