As an Autonomous Eye Care Specialist leasing space within our Sam's Club facilities, you will play a pivotal role in delivering comprehensive eye and vision care, alongside promoting overall health and wellness.

As an independent leaseholder, you will enjoy the freedom to:

Essential Qualifications for a lease agreement:
  • Doctor of Optometry (OD) from an accredited institution
    • Valid state optometry license
  • Ophthalmologist (MD or DO) from an accredited institution
    • Valid state ophthalmology license
  • General Liability Insurance Certificate
  • Professional Liability Insurance Certificate
  • W-9 form
