Job Title: Nurse Practitioner - Longitudinal Care Specialist

We are seeking a dedicated Nurse Practitioner to join our care team at Emcara Health. As a Longitudinal Care Specialist, you will play a pivotal role in providing comprehensive healthcare services to patients throughout their healthcare journey.

About the Role:
  • Conduct comprehensive in-home assessments for patients, including medical history reviews, physical examinations, and medication management.
  • Formulate accurate diagnoses and develop individualized treatment plans based on evidence-based practices and guidelines.
  • Manage and monitor chronic health conditions, such as diabetes, hypertension, and cardiovascular diseases, ensuring optimal disease control and patient education.
  • Prescribe medications and adjust dosages as necessary, while educating patients on proper medication administration and potential side effects.
  • Provide patient education on disease management, healthy lifestyles, and self-care practices to empower patients to take an active role in their health.
  • Collaborate closely with physicians, nurses, and other healthcare professionals to ensure coordinated care and seamless transitions for patients.
Requirements:
  • Active, unrestricted current licensure as a Nurse Practitioner in the state in which you will be providing care for patients.
  • Certification as a Family Nurse Practitioner or Adult-Gerontology Nurse Practitioner.
  • Obtain autonomous provider status within six months of meeting eligibility requirements.
  • Preferred two years of experience in a primary care or longitudinal care setting.
