Job Title: Family Support Specialist

Health Connect America is seeking a highly motivated and compassionate Family Support Specialist to join our team. As a key member of our clinical services team, you will provide best practice, quality clinical services to children and families in need.

Responsibilities:
  • Communicate effectively with clients, colleagues, and other stakeholders to provide support and guidance.
  • Develop and implement treatment plans in collaboration with the treatment team and Clinical Supervisor.
  • Provide individual, group, and family therapeutic interventions and skill building as outlined in the treatment plan.
  • Ensure all client documentation is completed within established time frames.
  • Attend and actively participate in weekly Treatment Team meetings.
  • Link clients and families with specific services and resources as identified in the treatment plan.
  • Serve as a liaison with other professionals and agencies as related to the client.
  • Actively recruit new referrals to ensure a full caseload.
  • Provide 24/7 on-call support for clients as per program requirements.
  • Ensure compliance with all state regulatory bodies and COA standards.
  • Participate in and complete all assigned training and professional development activities.
Qualifications:
  • Bachelor's Degree in a human services discipline, including Social Work, Psychology, Sociology, Counseling, or Criminal Justice, and 1 year of experience working with children, or BA/BS degree with 3 years of experience working with children.
