Rapid Rehousing Case Manager Role

Covenant House Texas seeks a dedicated Rapid Rehousing Case Manager to join our team in providing essential support services to homeless youth. As a key member of our staff, you will be responsible for case planning, implementation, and ensuring participants achieve sustainable, independent living.

  • Develop and implement comprehensive case plans in accordance with Covenant House Texas philosophy, policies, and procedures.
  • Conduct assessments to determine participant eligibility and develop individualized housing and service plans.
  • Provide case management services, including referrals, follow-ups, and progress tracking.
  • Collaborate with formal partner agencies and attend weekly multi-disciplinary team meetings.
  • Prepare financial assistance packets for submission to the appropriate agency.
Requirements
  • Bachelor's degree in social or behavioral sciences, administrative field, or related area; or 2 years of relevant experience with youth.
  • Experience collaborating with clients in crisis and strong ability to resolve and cope with immediate crises.
  • Knowledge of community resources and ability to maintain a positive and friendly attitude.
