About the Role

We are seeking a highly skilled Care Navigator to join our team at Carolina Complete Health. As a Care Navigator, you will play a critical role in developing and implementing personalized care plans for our members, ensuring they receive high-quality, cost-effective healthcare outcomes.

Key Responsibilities
  • Evaluate Member Needs: Assess the needs of our members, identify barriers to care, and recommend and facilitate plans for the best possible outcomes.
  • Develop Personalized Care Plans: Create or contribute to the development of personalized care plans that address the unique needs of our members.
  • Coordinate Care: Coordinate between members, families, caregivers, and healthcare providers to ensure timely access to care and services.
  • Monitor Progress: Monitor progress towards care plan goals and collaborate with healthcare providers to revise plans as needed.
  • Document and Maintain Records: Collect, document, and maintain all member information and care management activities to ensure compliance with regulatory requirements.
  • Perform On-Site Visits: Conduct on-site visits to assess member needs and collaborate with providers or resources as necessary.
  • Provide Education and Support: Educate care managers, members, and their families/caregivers on procedures, healthcare provider instructions, care options, referrals, and healthcare benefits.
Requirements
  • Education: Bachelor's degree required.
  • Experience: 2-4 years of related experience required.
  • Licensure: Current state clinical license preferred.
