Case Manager

ASHLIN Management Group, Inc. is seeking a skilled Case Manager to provide intensive case management services to at-risk or high-risk clients. The ideal candidate will possess a strong understanding of social work principles and be able to effectively communicate with clients and colleagues.
Key Responsibilities- Assist with scheduling and conducting assessments to identify client needs and develop individualized employment plans.
- Provide wrap-around support to clients, including referrals and follow-up consultation as needed.
- Develop and implement life skills and job readiness curriculum, ensuring cultural sensitivity and relevance.
- Identify and address barriers to service planning, making referrals and providing support as needed.
- Update client status promptly and coordinate actions with team members to ensure seamless service delivery.
- Schedule appointments, review employment goals, and make individual job matches in collaboration with job developers.
- Present employability workshops on life, job preparation, and job search skills, utilizing group discussions and individual skill practice to build client self-confidence.
- Counsel clients on barriers to employment and workshop attendance, ensuring all clients complete an employment application, custom resumes, and practice interviewing techniques.
- Assist clients in resolving scheduling problems for job search activities and ensure a minimum of 80% of participants are placed into unsubsidized full-time employment, making at least minimum wage, and remaining on the job through the first pay check.

- Ability to effectively communicate with superiors and subordinates in a professional manner.
- Articulate and expressive communication skills.
