We are seeking a skilled RN Case Manager to join our Hospice team at Assured Home Health and Hospice. As a Hospice RN, you will play a vital role in delivering high-quality end-of-life care to our patients and their families.
Key Responsibilities- Conduct initial nursing evaluations to determine patient eligibility for hospice services within 48 hours of referral.
- Identify and assess patients' physical, psychosocial, and environmental needs, with re-evaluations every 15 days as needed.
- Develop and implement individualized plans of care, documenting patient progress and response to care.
- Collaborate with patients, families, attending physicians, and interdisciplinary teams to provide comprehensive care.
- 1 year of clinical experience as a Registered Nurse.
- Current RN licensure in the state of practice and 1 year of clinical experience.
- Current CPR certification.
- Valid driver's license, vehicle insurance, and access to a reliable vehicle or public transportation.
