Job Summary:

The Residential Nurse provides medical case management for individuals with developmental disabilities living in community-based homes. This role involves supervising the completion of delegated nursing tasks, participating in agency training, and ensuring facility compliance with all physicians' orders.

Key Responsibilities:
  • Participate in the Interdisciplinary Team to provide nursing recommendations related to individuals' overall plans of care.
  • Monitor and coordinate the delivery of preventive and routine health care for individuals with developmental disabilities.
  • Adhere to and ensure facility compliance with all physicians' orders.
  • Ensure adequate scheduling and completion of health-related exams, appointments, laboratory/diagnostic testing as ordered or required.
  • Accompany individuals on appointments as deemed necessary.
  • Assist site management in coordinating medical program needs with community services, medical providers, and other related agencies.
  • Coordinate and provide health services training for staff.
  • Complete nursing responsibilities as indicated on the Nursing Responsibility Checklist for Residential Programs.
  • Monitor for completion of delegated health-related tasks to Direct Support Staff and LPNs.
  • Flex schedule to provide medication practicums as required by regulation for Direct Support Staff and LPN as needed or deemed necessary by RN Supervisor.
  • Supervise medication administration responsibilities completed by Direct Support Staff and LPNs, and ensure that staff who are passing medications are current in their staff medication errors.
  • Ensure that the plan of correction and retraining follows the Medication Error Point System.
  • Complete a thorough Semi Annual Medication review (Non-ICF) or ensure a Pharmacist completes a Quarterly Medication Review.
  • Complete (Quarterly-ICF) Nursing review on each individual.
  • Update the Plans of Nursing Services as health status and diagnosis change.
  • Within each month, complete a monthly breast exam on female individuals if ordered.
  • Utilize technology and software approved by the Agency to provide care planning and documentation related to individual care.
  • Provide phone triage to sites on caseload during working hours.
  • Flex schedule to provide services to individuals/sites as deemed necessary by the RN Supervisor.
  • Participate in various nursing workgroups as assigned.
  • Maintain compliance with all applicable State/Federal regulations and agency policy and procedure.
  • Communicate issues and carry out all directives from Nursing to the assigned site when notified that state or federal survey has arrived.
  • Participate in any plan of correction related to medical citations resulting from any internal or external survey.
  • Maintain communication with LPN (as applicable), Senior Residential Supervisor, residential managers, Service Coordination, Nursing Supervisor, and other staff as necessary.
  • Provide nursing coverage to all People Inc programs as directed by Nursing.
  • Provide agency training related to nursing services as required.
  • Comply with all agency policies and procedures.
  • Other duties as assigned by Nursing.


Qualifications:
  • Current NYS license to practice as a Registered Nurse.
  • NYS Drivers License that meets agency policy.
  • CPR certification and ability to perform CPR when necessary.
  • Ability to meet physical requirements of the position when performing the following: Standing, walking, sitting, bending, stooping, squatting, kneeling, and climbing.
