Job Summary

We are seeking a highly skilled and experienced Medical Director to join our team. As a Medical Director, you will be responsible for overseeing the medical operations of our skilled nursing facilities, providing high-quality patient care, and collaborating with our Nurse Practitioners and Physician Assistants to manage day-to-day resident care.

Key Responsibilities
  • **Medical Directorship**: Serve as the medical director for skilled nursing facilities, participating in administrative and quality initiatives, and providing guidance to facility staff.
  • **Patient Care**: Provide high-quality, patient-centered, comprehensive primary care services by rounding on patients residing in SNFs, diagnosing and treating patients with acute illness, and managing chronic diseases and conditions.
  • **Collaboration**: Collaborate with Nurse Practitioners and Physician Assistants to manage day-to-day resident care, and supervise collaborating nurse practitioners and physicians assistants.
  • **Documentation**: Complete provider documentation in Theoria's proprietary electronic health record (EHR) system, ChartEasyTM, and utilize electronic/online medical tools such as EHR systems and state/county medical systems.
  • **Quality Initiatives**: Participate in Theoria's Chronic Care Management, Transitional Care, RPM, and other medical quality initiatives and programs.
Requirements
  • **Medical Licensure**: Must be able to supervise collaborating nurse practitioners and physicians assistants, and utilize electronic/online medical tools.
  • **Experience**: Must have experience as a Medical Director or in a similar leadership role, with a strong background in primary care and post-acute care.
  • **Skills**: Must have excellent communication and leadership skills, with the ability to work effectively in a team environment.
