Job Summary:
We are seeking a compassionate and skilled Registered Nurse to join our team as a Hospice Nurse. As a Hospice Nurse, you will provide end-of-life care to patients and their families, ensuring their comfort and quality of life. You will work collaboratively with a team of healthcare professionals to develop and implement individualized care plans that meet the unique needs of each patient.
Responsibilities:
- Conduct thorough assessments of patients and their families to identify their needs and develop individualized care plans.
- Coordinate care with other healthcare professionals, including physicians, therapists, and social workers.
- Provide emotional support and education to patients and their families, helping them to navigate the end-of-life care process.
- Ensure that patients and their families receive high-quality, compassionate care that meets their unique needs.
- Collaborate with the interdisciplinary team to develop and implement care plans that meet the needs of patients and their families.
Requirements:
- Current RN license in the State of Florida.
- BSN degree from an accredited college or university.
- Hospice certification preferred.
- Basic Life Support Certification.
- One year of experience in hospice care or a related field.
Preferred Qualifications:
- Bilingual (English/Spanish) preferred.
- Basic computer skills and EMR software experience a plus.
