About the Role

This LPN Care Manager position at Knute Nelson is a challenging and rewarding opportunity to make a meaningful impact on the lives of our clients. As a key member of our care management team, you will be responsible for coordinating client care and services to improve the quality of life for those we serve.

**Job Summary:**
Schedule: Full-time | 64 hours per pay period | Monday-Friday | 8 am – 4:30 pm

As a Care Manager with Knute Nelson, you will support at-risk clients with aging care and service needs by promoting effective education, age-well self-management support, timely care, and service delivery to achieve optimal quality and financial outcomes. Your expertise will be essential in ensuring that professionalism is practiced at all times, including complying with legal and ethical standards, privacy requirements, client advocacy, conflicts of interest, and professional boundaries.

You will participate in developing models of care and support to integrate social and clinical pathways for whole-person care delivery within Knute Nelson and in partnership with other providers and payers. Collaboration with members of the Knute Nelson organization and other partnering providers and organizations will also be an important part of your role. Commitment to excellence in improving the health status of the aging community will drive success.

**Key Responsibilities:**
• Implement programming for assigned clients based on their physical, psychosocial, emotional, and spiritual needs.
• Complete client health screenings and evidence-based practice guidelines to identify health, well-being, environmental concerns, and social determinants of health.
• Assess and identify client strengths, needs, concerns, and preferences.
• Interpret client needs and translate them into client care plans that identify goals and resources to facilitate wellness.
• Establish goals and a plan of care or service for assigned clients.
• Implement care and service plans based on qualified assessments.
• Advocate for an individual's healthcare needs and coordinate care to minimize healthcare fragmentation.
• Manage and monitor ongoing care and service provision and need for assigned clients.
• Assist the technology team and implement technology deployment and usage strategies.
• Coordinate care and services with other healthcare and service provider(s)/organizations.
• Practice care management professionally.
• Demonstrate comprehension and application of theories related to human behavior, crisis, family systems, personality, change, and adult learning.
• Participate in meetings and collaborative planning related to population health strategy.

The estimated salary for this position is $65,000 - $75,000 per year.
