Kettering Health is seeking a highly skilled Care Manager II - Acute Care to join our team. As a Care Manager II, you will play a critical role in supporting and facilitating coordinated care delivery to patients, ensuring seamless transitions and optimal outcomes.

Key Responsibilities
  • Assist the care team in identifying and eliminating barriers to discharge, ensuring timely and efficient care delivery.
  • Collaborate with the care team to develop and implement effective post-acute care plans, prioritizing moderate and high-risk patients with complex health needs.
  • Provide advocacy, information, and referral services to patients and families, addressing their psychosocial needs and promoting clear communication with the care coordination team and caregivers.
  • Participate in data collection, health outcomes reporting, clinical audits, and programmatic evaluation to drive organizational goals for length of stay, readmissions, and patient experience.
Requirements
  • Masters of Social Work or related field required.
  • Current Ohio Social Work license required.
  • Clinical experience in a healthcare setting, with awareness of developmental needs across the age span.
  • Ability to work effectively in multidisciplinary and independent settings, communicating well verbally and in writing.
  • Ability to coordinate work with other hospital disciplines and recognize the dynamics of individuals from diverse populations.
