Job Summary

The Clinical Care Specialist plays a vital role in the delivery of high-quality patient care at the Northern Tier Center for Health-Richford Health. This position works closely with medical providers and support staff to provide clinical assistance and ensure the effective delivery of care.

Responsibilities include interacting with patients and members of the care team, performing patient intake, updating medical data, and triaging medical concerns. The ideal candidate will possess strong communication and interpersonal skills, as well as the ability to juggle multiple tasks and prioritize workload.

Key Responsibilities:

* Interact with patients and members of the care team to provide a range of care services
* Perform patient intake, including vital signs
* Update medical data, medications, and health problems in charts
* Triage medical concerns
* Assist medical providers during procedures and exams
* Handle referrals and phone calls to other provider offices for appointments
* Collect specimens for outside laboratory testing and perform basic in-house laboratory tests
* May execute organization-approved standing orders per written procedure
* May initiate and/or execute age-appropriate preventive care measures (within scope of practice) as detailed in the Lifecycle Milestone Policy
* Engage patients in self-management action planning
* Assist Provider with patient education and instructions, self-management tools, and counseling on healthy behaviors
* Effectively communicate with patients using techniques such as motivational interviewing when appropriate

Requirements:

* Bachelors Degree in Nursing preferred
* Ability to perform venipuncture, administer injections, and complete CLIA waived lab tests
* Knowledge of correct specimen handling procedures
* Knowledge of medical terminology
* Knowledge of OSHA manual and standards, including appropriate hazardous waste handling and disposal
* Knowledge of billing codes and procedures
* Ability to function within a team environment to achieve stated objectives
