Job Title: Night RN CAHC

Job Summary:

We are seeking a compassionate and skilled Registered Nurse to join our Hospice team as a Night RN CAHC. As a key member of our interdisciplinary team, you will provide high-quality, patient-centered care to individuals with terminal illnesses and their families.

Responsibilities:

  • Conduct initial assessments of patients and families to determine hospice needs and develop individualized care plans.
  • Provide professional nursing care, including administering medications, treatments, and performing wound care and dressing changes as needed.
  • Collaborate with the interdisciplinary team to identify patient and family needs and develop strategies to meet those needs.
  • Communicate effectively with patients, families, and healthcare team members to ensure seamless care coordination.
  • Document patient information accurately and thoroughly in the electronic medical record.
  • Participate in quality improvement initiatives and contribute to the development of policies and procedures.

Requirements:

  • Graduate of an accredited nursing program.
  • Current RN licensure in the Commonwealth of Virginia.
  • Minimum of 2 years of hospice or home health experience, with a focus on terminal care.
  • Ability to work independently and as part of a team.
  • Excellent communication and interpersonal skills.
  • Basic computer skills and proficiency in electronic medical records.

Working Conditions:

The physical demands of this position include standing, sitting, and walking for extended periods. The ability to lift and move up to 50 pounds is required. Regular use of a computer screen and telephone is necessary. Ability to drive and negotiate steps is also required.
