Responsibilities- Conduct thorough assessments of patients and families to identify their unique needs and develop personalized care plans.
- Provide expert nursing care, including administering medications, treatments, and performing various procedures as prescribed.
- Collaborate with the healthcare team to ensure seamless care transitions and effective communication.
- Document patient progress and outcomes accurately and in a timely manner.
- Participate in quality improvement initiatives and contribute to the development of best practices in hospice care.
- Graduate of an accredited nursing program with a current RN license in the Commonwealth of Virginia.
- Minimum of 2 years of experience in hospice, home health, or ICU/CCU settings.
- CHPN certification preferred.
- Excellent communication and interpersonal skills, with the ability to work effectively in a team environment.
