LPN Hospice Nurse

We are seeking a compassionate and skilled Licensed Practical Nurse (LPN) to join our team at Hospice of the Piedmont Inc. as a Hospice LPN Nurse. In this role, you will provide direct patient care to our residents, working closely with our staff and volunteers to ensure the highest level of care and comfort.
Responsibilities:- Provide direct patient care to Hospice residents, including administering medications, wound care, and other nursing interventions.
- Contribute to the development and implementation of patient care plans, working collaboratively with the healthcare team.
- Document patient information accurately and thoroughly, maintaining confidentiality and adhering to HIPAA guidelines.
- Assist with food preparation and housekeeping tasks as needed, ensuring a clean and safe environment for our residents.
- Participate in staff training and education, staying up-to-date on the latest nursing practices and procedures.
- Communicate effectively with patients, families, and healthcare team members, providing emotional support and guidance as needed.
- Collaborate with the House Manager to maintain a safe and healthy environment, adhering to infection control protocols and other safety measures.
- Conduct clinical observations and evaluations of CNAs and other house staff, providing constructive feedback and coaching as needed.
- Provide guidance and direction to employees, ensuring they receive the necessary training and support to excel in their roles.
- Support the Individual Achievement Review and Evaluation (IAR&E) performance evaluation process, providing regular feedback and coaching to staff members.
- Adhere to HOP personnel policies and procedures, including recruitment and hiring processes, accident reports, and other administrative tasks.
- Complete other assignments and tasks as requested by management, prioritizing tasks and managing time effectively.
- High School Graduate or GED
- Graduation from an accredited school of practical nursing
- Licensed as a Licensed Practical Nurse in the Commonwealth of Virginia
- Two years relevant nursing experience required
- Ability to manage all aspects of terminal care, health team service coordination, and documentation
- Nursing home/assisted living facility experience preferred
- Basic knowledge of Medicare/Medicaid reimbursement regulations
- Emotionally mature and caring individual willing to give support and assistance to terminally ill and their families
- Basic to intermediate proficiency in word processing software helpful
- Ability to effectively present information in one-on-one and small group situations to clients and other employees of the organization
- Ability to write reports, correctly reflect treatment and document client progress
- Good oral and written communication skills
- Skill in establishing and maintaining effective working relationships
- Must be flexible and able to function in stressful situations
- Incumbent should have the ability to manage multiple tasks
- Ability to effectively communicate and work with family members, staff and management
- Ability to work with and maintain confidentiality of patient data
- The ability to work as a team member is necessary for this position.
- Due to the nature of services provided, it may be necessary at times for employees to work beyond scheduled work hours to ensure adequate patient care
