Job Summary:The LPN Hospice Nurse will provide direct patient care to Hospice residents, working closely with the staff and volunteers to ensure the highest level of care and comfort. This role requires a strong understanding of nursing principles, excellent communication skills, and the ability to work effectively in a team environment.
Responsibilities:- Provide direct patient care to Hospice residents, including administering medications, wound care, and other nursing interventions.
- Contribute to the development and implementation of patient care plans, working closely with the interdisciplinary team.
- Document patient information accurately and thoroughly, maintaining confidentiality and adhering to HIPAA guidelines.
- Assist with food preparation and housekeeping tasks as needed to maintain a clean and safe environment for residents.
- Collaborate with the House Manager to ensure compliance with health, fire, and safety regulations.
- Participate in clinical observations and evaluations of CNAs and other house staff, providing feedback and guidance as needed.
- Provide guidance and direction to employees, ensuring they receive necessary training and support.
- Support the Individual Achievement Review and Evaluation (IAR&E) performance evaluation process, providing coaching and feedback to enhance individual performance.
- Follow HOP personnel policies and procedures, including recruitment and hiring processes, accident reports, and other relevant guidelines.
- Complete other assignments and tasks as requested by management.
- High School Graduate or GED
- Graduation from an accredited school of practical nursing
- Licensed as a Licensed Practical Nurse in the Commonwealth of Virginia
- Two years relevant nursing experience required
- Ability to manage all aspects of terminal care, health team service coordination, and documentation
- Nursing home/assisted living facility experience preferred
- Basic knowledge of Medicare/Medicaid reimbursement regulations
- Emotionally mature and caring individual willing to give support and assistance to terminally ill and their families
- Basic to intermediate proficiency in word processing software helpful
- Ability to effectively present information in one-on-one and small group situations to clients and other employees of the organization
- Ability to write reports, correctly reflect treatment and document client progress
- Good oral and written communication skills
- Skill in establishing and maintaining effective working relationships
- Must be flexible and able to function in stressful situations
- Incumbent should have the ability to manage multiple tasks
- Ability to effectively communicate and work with family members, staff and management
- Ability to work with and maintain confidentiality of patient data
- The ability to work as a team member is necessary for this position.
- Due to the nature of services provided, it may be necessary at times for employees to work beyond scheduled work hours to ensure adequate patient care
- Must demonstrate basic computer skills.
- Driver's license, personal automobile and current automobile liability insurance
- Frequent travel may be required
