Behavioral Health Specialist

INTEGRIS Health, Oklahoma's largest not-for-profit health system, is seeking a skilled Primary Care Behavioral Health Consultant to join our team in Oklahoma City, OK.
This is a Monday - Friday, day shift position that offers the opportunity to work collaboratively with our primary care team to provide exceptional care to patients with medical and behavioral health needs.

Responsibilities- Work with the primary care team to identify, triage, and manage patients with medical and behavioral health problems efficiently and effectively.
- Assist in detecting at-risk patients and preventing further psychological or physical deterioration.
- Use brief assessment tools to identify concerns, triage, and outcome measures.
- Develop interventions and set goals for behavioral modification.
- Operate within the provider care team, evaluating patient care plans with primary care team members.
- Connect with new and established patients regarding integrated care models and care team information.
- Assist in preventing relapse or morbidity in conditions that tend to recur over time.
- Teach patients, families, and care team members prevention and treatment strategies using motivational interviewing and teach-back techniques.
- Accurately record patient history, exam notes, medication history, and ongoing care in the medical record, referring patients for ongoing mental health treatments as needed.
- Consult with PCPs to enhance understanding of patient needs, provide decision support for treatment planning, and assist in implementing and monitoring biopsychosocial treatment plans.
- Must be able to communicate effectively in English.
- Requires a master's degree in social work, psychology, counseling, or a related field.
- Licensed LPC, LCSW, LMFT, LBP, or other Licensed Mental Health Professional in the state of Oklahoma.
- Previous professional experience is preferred.
- Behavioral health experience is preferred.
- Experience in a medical setting is preferred.
- Excellent interpersonal skills and ability to work collaboratively with primary care staff, patients, and other behavioral health clinicians.
