Key Responsibilities- Medication Management: Accurately prepare and dispense medications for hospitalized patients, ensuring correct dosages, routes of administration, and timing according to physician orders.
- Clinical Consultation: Collaborate with healthcare teams, including doctors and nurses, to provide drug therapy recommendations, monitor patient medication regimens, and adjust dosages based on patient response.
- Medication Reconciliation: Conduct thorough medication reconciliation during patient admissions, transfers, and discharges to prevent medication errors and ensure continuity of care.
- Patient Education: Provide counseling and education to patients on their medications, including potential side effects, interactions, and the importance of adherence to their prescribed therapy.
- Pharmacist License: Hold an active California (CA) Pharmacist License.
- Education: Possess a PharmD degree.
- Experience: Have solid inpatient pharmacy experience.
- EPIC Experience: Prior experience with EPIC is a plus.
