Physical Therapist

The City and County of San Francisco is seeking a highly skilled Physical Therapist to join our team at the San Francisco Department of Public Health. As a Physical Therapist, you will play a critical role in providing physical therapy evaluations and treatments to a diverse client population.
Key Responsibilities- Perform physical therapy evaluations, including assessment of functional status and physical condition
- Develop and implement physical therapy treatment plans, including goal setting
- Administer appropriate physical therapy interventions and procedures, including education and training of caregivers
- Communicate effectively with other care team members and patients
- Document patient information and treatment plans according to regulatory and practice guidelines
- Meet administrative requirements, including attendance of staff meetings and record-keeping
- Oversee care provided by physical therapy assistants and certified home health aides
- Participate in patient care meetings and performance improvement projects
- Registration as a Physical Therapist licensed by the Physical Therapy Board of California
- Basic Life Support for Professional Rescuer (BLS) Certificate issued by the American Heart Association
