Position Overview

Direct Care Worker

Key Responsibilities
  • Assist patients with personal care tasks as outlined in their care plan, including hygiene, mobility, and elimination needs. Report any challenges to the RN or Director of Clinical Services promptly.
  • Maintain a safe environment for patients, utilizing equipment properly and safely.
  • Implement Universal Infection Control Precautions during all patient interactions.
  • Support patients and families with home maintenance tasks as specified in the care plan, such as light cleaning and meal preparation.
  • Monitor and record vital signs and changes in patient condition, ensuring accurate reporting.
  • Employ safe techniques for transferring and positioning patients.
  • Identify when additional support from the home health team is necessary and communicate this to the RN or Director of Clinical Services.
  • Respect patient confidentiality and autonomy at all times.
  • Document care provided and patient responses accurately and submit reports in a timely manner.
  • Work within the scope of your training and experience.
  • Adhere to all local, state, and federal regulations, including Medicare guidelines and agency policies.
  • Manage your caseload efficiently while delivering high-quality care.
  • Consistently meet or exceed the service standards set by 365 Health Services.
  • Conduct all professional activities with integrity and ethical standards.
  • Utilize supplies and equipment effectively to minimize waste.
  • Complete a minimum of 12 hours of in-service training annually.
  • Maintain regular attendance and punctuality.
  • Follow the Code of Conduct established by 365 Health Services.
  • Engage positively with all staff members, supporting the mission and vision of 365 Health Services.
  • Ensure compliance with agency policies regarding confidentiality and HIPAA regulations.
Employment Type

Compensation

Hourly Rate: $14.00
