Position: Care Management Support Coordinator II

Location: Remote

Duration: 4 Months

Pay Rate: $20.00/Hr. on W2


Position Overview

This role involves assisting with administrative care management tasks, which include outreach efforts, responding to incoming calls, and coordinating services. You will act as a liaison for members, providers, and internal staff to address concerns and maintain member records in compliance with current state and regulatory standards.

Key Responsibilities:

  • Assist in care management activities, ensuring effective communication with members and providers.
  • Conduct outreach to members to facilitate care plan follow-ups and provide information on community resources.
  • Document and maintain non-clinical member records, adhering to regulatory requirements.
  • Support member onboarding and manage daily administrative tasks, including sending welcome letters and educational materials.
  • Perform outreach via phone to assigned members, ensuring high engagement rates.
  • Maintain accurate records of member interactions and follow up as necessary.
  • Comply with all organizational policies and standards.
  • Engage in activities related to the medical and psychological aspects of coordinated care.

Required Qualifications:

  • 2-3 years of experience in a managed care or medical office environment.
  • Strong understanding of customer service principles and practices.
  • Proficient in operating office equipment, including personal computers.
  • High School diploma or GED required; 1-2 years of relevant experience preferred.

Preferred Skills:

  • Excellent communication abilities.
  • Familiarity with medical terminology.
  • Strong organizational and time management skills.
