Job Responsibilities:
The Registered Nurse Case Manager contributes to our Mission of Caring by providing compassionate direct patient care to our clients in their homes throughout Rhode Island. The RN Case Manager conducts day-to-day nursing activities, including assessment and education, in a one-on-one setting in the client's home.
Requirements:
- Current and valid RN license in Rhode Island
- Home care experience preferred
- Current and valid CPR certification
- Reliable transportation, valid driver's license, automobile insurance
