Job Summary:

As a Cardiac Care Coordinator, you will be responsible for providing exceptional patient care and support to individuals with complex heart-related concerns. You will work closely with our multidisciplinary team to coordinate patient visits, gather required information, and assemble clinical data for thorough evaluation.

Key Responsibilities:

• Assist patients and their caregivers through the continuum of care

• Coordinate patient visits and gather required information

• Assemble clinical data for thorough evaluation

• Coordinate additional testing procedures as directed by the team or treating physician

• Educate patients about their treatment plans and provide community education services

Requirements:

• Graduate from an accredited school of nursing or Physician Assistant Program

• Two years of nursing experience (if RN) or a master's degree from a PA or NP program (may be substituted for nursing experience)

• Current RN license in the state of Georgia or certification to practice as a Physician Assistant in the State of Georgia

Preferred Qualifications:

• Experience in working with cardiac disease populations, specifically in advanced heart failure, EP, or structural heart

• Knowledge and experience in using the EMR system

Compensation and Benefits:

We offer a competitive salary range of $65,000 - $85,000 per year, depending on experience and qualifications. In addition, you will be eligible for a comprehensive benefits package, including health insurance, retirement savings, and paid time off.
