Job Title: Care Coordinator RN

Northside Hospital is a leading healthcare provider in Atlanta, committed to delivering exceptional patient care and advancing the quality of our services. As a Care Coordinator RN, you will play a vital role in ensuring seamless transitions for our patients and families.

Key Responsibilities:
  • Collaborate with patients, families, physicians, and hospital staff to determine the most appropriate setting of care and assess post-hospital needs.
  • Provide interventions to ensure effective and efficient clinical resource management.
  • Assess and address adjustment to hospitalization, crisis intervention, and psychosocial needs.
Requirements:
  • Graduate of an accredited Registered Nursing School (RN) with a current GARN license.
  • One (1) year previous experience in case management, utilization review, or social services.
  • Excellent oral and written communication skills.
  • Ability to type accurately at an approximate rate of 40-45 wpm.
Preferred Qualifications:
  • BSN
  • Three (3) years' experience in case management, social services, or related field.
  • Certification as Certified Case Manager.
