Care Navigator

We are seeking a highly skilled and compassionate Care Navigator to join our team at Klamath Basin Behavioral Health. As a Care Navigator, you will play a critical role in supporting individuals with psychiatric, substance abuse, medical, social, and legal issues to improve their daily living skills and achieve their recovery goals.
Key Responsibilities- Build and maintain working relationships with clients, therapists, psychiatric providers, community partners, and medical providers to ensure comprehensive care.
- Develop and implement individualized plans to address client needs and goals.
- Provide case management and skills-building services to support clients in achieving their treatment plans.
- Collaborate with the multidisciplinary team to ensure timely referrals to internal and external resources.
- Document client progress and maintain accurate records in accordance with federal, state, and local regulations.
- Bachelor's degree in a behavioral science field or a related field.
- At least three years of relevant work experience, education, training, or experience in a behavioral health setting.
- Valid Oregon driver's license and insurable driving record.
- Personal automobile insurance required if clients are transported or agency business is conducted using a personal vehicle.
- Competitive salary range: $25.10 DOE.
