About the Role:
We are seeking a compassionate Social Worker to join our EverHeart Hospice team. As a key member of our interdisciplinary team, you will provide essential support to patients and families navigating end-of-life care. This role offers a unique opportunity to make a meaningful difference in the lives of others.

Key Responsibilities:
  • Develop and implement personalized care plans that address the unique needs of each patient and family.
  • Collaborate with healthcare professionals to ensure seamless care transitions and effective communication.
  • Provide emotional support and guidance to patients and families during challenging times.

Requirements:
  • Master's Degree in Social Work (MSW) from a social work curriculum accredited by the Council on Social Work Education.
  • Current Social Work licensure in the State of Ohio.
  • Minimum of two years of experience in a clinical setting.

Estimated Salary: $65,000 - $75,000 per year.
