As a key member of our interdisciplinary team, you will play a vital role in providing high-quality psychiatric rehabilitation services to our residents.
Responsibilities:
- Assist in the planning, development, and implementation of psychiatric rehabilitation services programs.
- Conduct assessments and develop care plans for residents in collaboration with the interdisciplinary team.
- Provide individual and group counseling, training, and behavioral interventions to promote resident skills in self-maintenance, social functioning, and community living.
- Monitor resident progress and make recommendations for adjustments to care plans as needed.
- Collaborate with community agencies and other stakeholders to ensure seamless transitions and continuity of care.
- Participate in surveys and inspections made by authorized government agencies.
Requirements:
- Bachelor's degree in a human services field (e.g., sociology, special education, rehabilitation counseling, or psychology).
- Minimum of one year of supervised experience in mental health or human services.
- Ability to establish working relationships with residents, family members, staff, visitors, and government agency personnel.
- Strong communication and interpersonal skills.
- Ability to work cooperatively and harmoniously in a fast-paced environment.
