We are seeking a dedicated and compassionate individual to join our team as a Psychiatric Rehabilitation Services Coordinator. As a member of our community-driven organization, you will have the opportunity to make a meaningful difference in the lives of our residents.

Key Responsibilities:

  • Review and assist residents in understanding their treatment plans.
  • Prepare and assist residents with active participation in treatment plan reviews.
  • Provide and/or coordinate the delivery of psychiatric rehabilitation services programs.
  • Re-evaluate residents on a quarterly basis.

Requirements:

  • Bachelor's or Master's degree in a human service profession (e.g., social work, psychology, sociology, counseling psychology).
  • Demonstrated sensitivity, ability, and skills in working with and understanding the needs of residents, family members, staff members, and personnel from community agencies.
  • Demonstrated familiarity and knowledge of current trends in the rehabilitation of the mentally ill.
  • The ability to present concise, meaningful written reports and articulate the social/psychological needs of residents.
  • Familiarity with paperwork related to this role (e.g., MDS, care plans, progress notes).
