Job SummaryWe are seeking a skilled and compassionate Social Worker to join our team. As a Social Worker, you will work with adolescents and families in foster care to provide case management and clinical services. You will be responsible for conducting intake and assessment interviews, developing and implementing treatment plans, and providing short-term therapy to individuals and families. You will also work collaboratively with other professionals to ensure consistent service planning and delivery.
Key Responsibilities- Conduct intake and assessment interviews to identify the needs and strengths of adolescents and families
- Develop and implement individualized treatment plans to promote permanency, safety, and well-being
- Provide short-term therapy to individuals and families to address trauma, attachment, and other mental health concerns
- Collaborate with other professionals to ensure consistent service planning and delivery
- Participate in interdisciplinary treatment team meetings and service plan reviews
- Document all interactions and interventions in accordance with agency policies and procedures
- Masters degree in Social Work and licensure (LMSW) or licensure ready required
- Experience working with adolescents and families in foster care preferred
- Bilingual English/Spanish preferred
- Ability to establish working relationships with clients in need of supportive services
- Proven competence in written, verbal, organizational, and prioritization skills
