Job Overview

Key Responsibilities:

  1. Client Assessment and Evaluation:
    • Conduct thorough assessments of clients' needs, strengths, and challenges.
    • Develop personalized service plans based on assessment findings.
  2. Case Management:
    • Coordinate and monitor services provided to clients, ensuring they receive appropriate care and support.
    • Maintain regular contact with clients to evaluate progress and adjust service plans as needed.
  3. Crisis Intervention:
    • Provide immediate support and intervention during crisis situations.
    • Develop safety plans and connect clients with emergency resources.
  4. Advocacy and Resource Connection:
    • Advocate for clients' rights and access to services.
    • Assist clients in navigating social service systems and accessing resources such as housing, healthcare, financial assistance, and education.
  5. Counseling and Support:
    • Provide individual, family, and group counseling to help clients cope with challenges.
    • Offer emotional support and guidance to clients dealing with trauma, loss, and other life changes.
  6. Community Outreach and Education:
    • Conduct outreach activities to identify and engage underserved populations.
    • Develop and deliver educational programs and workshops on relevant social issues.
  7. Collaboration and Coordination:
    • Work closely with healthcare providers, schools, law enforcement, and other community organizations to coordinate care and services.
    • Participate in multidisciplinary team meetings to discuss client cases and develop comprehensive care plans.
  8. Documentation and Reporting:
    • Maintain accurate and confidential client records, documenting assessments, service plans, and progress notes.
    • Prepare reports for supervisors, funding agencies, and other stakeholders as required.
  9. Professional Development:
    • Stay informed about current social work practices, policies, and regulations.
    • Participate in continuing education and professional development opportunities.

Qualifications:

  • Education: Bachelor's degree in Social Work (BSW) required; Master's degree in Social Work (MSW) preferred.
  • Licensure: Current state licensure as a Social Worker (LSW or LCSW) required.
  • Experience: Minimum of 2 years of experience in social work or related field, with experience in case management and counseling preferred.
