Key Responsibilities:


Under the guidance of the Department manager or designated personnel, the specialist enhances rational and economical drug therapy for both inpatient and outpatient settings through active involvement in the medication decision-making process alongside physicians and other healthcare providers.

Mentors Pharmacist Residents, Interns, Pharmacy Technicians, and other support staff.
Conducts thorough medication history assessments upon referrals from healthcare professionals.

Formulates a treatment strategy based on diagnoses, which may encompass pharmacotherapy and lifestyle adjustments, ensuring seamless transitions across various care settings.

Identifies, addresses, and prevents potential and actual medication-related issues, including: unaddressed indications, inappropriate drug choices, suboptimal dosages, overdoses, medication non-adherence, adverse drug reactions, drug interactions, and unnecessary medications.

In accordance with protocols, initiates, modifies, monitors, and suggests alterations in drug therapy based on insights gained from patient interviews, objective monitoring metrics, and reviews of medication profiles and medical records.

Coordinates and interprets laboratory evaluations related to drug therapy, including serum drug concentrations, electrolytes, and other approved laboratory tests.

Monitors patients to assess and enhance outcomes, drug utilization, and adherence, while identifying and mitigating adverse drug reactions and eliminating redundant medication use.

Provides patient education regarding individual health conditions, including expected effects, side effects, the importance of adherence, and any specific instructions.

Ensures continuous improvement in the quality of pharmaceutical care services offered.
Delivers educational sessions to healthcare providers and staff through presentations, newsletters, formal reviews, and individual interactions.
Performs additional responsibilities as needed.

Minimum Qualifications:
Experience

Completion of a clinical pharmacy residency or equivalent clinical experience in direct patient care.
Education

B.S. degree in Pharmacy or Pharm.D. required.
Licensure, Certification, Registration

Pharmacist License (California) required upon hire.
Basic Life Support certification from the American Heart Association.

Additional Qualifications:
Thorough understanding of current pharmacological and biopharmaceutical principles, medical terminology, pathologies, disease states, and relevant information for medical management.
Proficient in aseptic techniques and preparation of sterile IV products and chemotherapy agents. Familiarity with emergency medications and drug monitoring.
Knowledge of drug utilization reviews and evaluations.
Awareness of Federal and State regulations governing pharmaceutical services.
Exceptional verbal and written communication skills.
Ability to operate pharmacy computer systems and related equipment. Proficient in inputting prescription labels.
Must obtain an NPI number within one month of employment.
Ability to work in a Labor/Management Partnership environment.
ACLS certification may be necessary in certain areas.
