Job Title: LPN Admission Screener

About The Position:
The McGuire Group Inc. is seeking a skilled LPN to join our team as an Admissions Screener in western New York. As a member of our admissions department, you will work closely with referral sources, facilities, and other teams to coordinate medical and social assessments, admissions, and transfers.

Responsibilities:
  • Complete admission assessments and recommend patients for long-term or subacute care
  • Visit designated hospital assignments daily
  • Knowledge of age-specific developmental factors specific to adult and geriatric residents (i.e., physical, cognitive, and socialization factors) in carrying out the admissions process
  • Interact with residents and family members, co-workers, clinical, and ancillary staff in a non-judgmental, supportive, and calm manner

Requirements:
  • Degree in Nursing, Social Work, or Physical Therapy preferred
  • Experience in Admissions Screening and long-term care is preferred
  • Good understanding of Medicare and other Managed Care insurance helpful
  • Must be flexible, professional, and reliable
  • Computer skills including keyboarding and PCC
  • Strong customer service skills
  • Valid New York State Driver's License and have transportation available for daily job-related travel
