About The Company:
The McGuire Group Inc. is a leading healthcare provider in western New York, comprised of five skilled nursing and subacute rehab facilities. Our mission is to deliver exceptional patient care and provide a supportive work environment for our team members.

About The Role:
We are seeking an experienced LPN to join our admissions team as an Admissions Screener. This role involves working closely with referral sources, facilities, and other teams to coordinate medical and social assessments, admissions, and transfers.

Key Responsibilities:
  • Complete admission assessments and recommend patients for long-term or subacute care
  • Visit designated hospital assignments daily
  • Knowledge of age-specific developmental factors specific to adult and geriatric residents (i.e., physical, cognitive, and socialization factors) in carrying out the admissions process
  • Interact with residents and family members, co-workers, clinical, and ancillary staff in a non-judgmental, supportive, and calm manner

Requirements:
  • Degree in Nursing, Social Work, or Physical Therapy preferred
  • Experience in Admissions Screening and long-term care is preferred
  • Good understanding of Medicare and other Managed Care insurance helpful
  • Must be flexible, professional, and reliable
  • Computer skills including keyboarding and PCC
  • Strong customer service skills
  • Valid New York State Driver's License and have transportation available for daily job-related travel

Compensation:
$28-$32 per hour, paid time off, health insurance, dental insurance, life insurance, disability insurance, vision insurance, and more
