Job Summary

We are seeking an LPN MDS Coordinator to join our team at Casa Del Sol Care Center in Las Cruces, NM. As a key member of our interdisciplinary team, you will be responsible for directing the nursing center's Resident Assessment Instrument (RAI) process, developing individualized resident care plans, and training new team members on clinical content and electronic completion of MDS items.

Responsibilities
  • Direct the nursing center's RAI process, ensuring accurate and timely completion of MDS and CAAs.
  • Develop and implement individualized resident care plans, identifying resident needs and projected outcomes.
  • Train new team members on clinical content and electronic completion of MDS items.
  • Participate in the Resident Assessment Process, ensuring accurate and timely completion of MDS and CAAs.
Requirements
  • Current LPN licensure in good standing.
  • Previous experience in a hospital, long-term facility, or other healthcare-related facility.
  • Knowledge of general, rehabilitative, and restorative nursing and medical practices and procedures.
  • Computer skills, including Windows applications.
  • AccuMed experience preferred.
